Yeah, I assume that's most cases - but even so . . . even a hundred ships all stuck together wouldn't be that big.

How many kilometres wide are the largest cities in the world today?  Counting the suburbs, maybe 40 kilometres at the most?  Space ships in 40k might be big, but they aren't on that scale.
If you think about it, a small escort is 2 - 5 km long.  I don't have my BFG books handy, but if that is the scale of the smallest ships, imagine the size of battleships.  Now imagine the size of several battleships being molded into one massive vessel after being lost in the warp

I would probably put hulks somewhere between 20 - 60 km tops. A single Battleship is around 10 km so to have the length between two and six times that sounds reasonable for hulks in my opinion.

That would not be a small escort at all given that cruisers are around those sizes. An escort is smaller than 2 km and a small escort would, of course, be even smaller.

The original Space Hulk rulebook stated that Hulks were anywhere in size between a "small moon and a large planet"

Given the smallest moon in our Solar System is 11km diameter and the largest moon is nearly 5300km in diameter I don't think 400km is a bad size
You have to bear in mind it's not just the ships which constitute hulk, it's asteroids, general debris and all manner of crap floating in space
